What the template includes
- Property, landlord, lease start date and monthly rent.
- A table of required one-time and recurring fees with amount, timing, refundability and notes, pre-filled with common fee names.
- Fees that only apply in some cases, such as late fees and pet fees.
- Optional add-ons the tenant may decline.
- A utilities grid: included in rent, paid to the provider, or billed by the landlord.
- A statement citing 765 ILCS 705/35 and lines for tenant and landlord initials.
How to use it
- Remove any fee the law bans. Check a fee if unsure.
- Fill in every fee a tenant must pay, with amounts.
- Place it as page one of the lease, not as an addendum.
- Use the same numbers in your listing, or link a free fee disclosure page that shows them.
Prefer it filled in automatically? The fee page builder prints the same table with your numbers, a QR code and a dated online copy for listings.
Frequently asked questions
Is this template enough to comply with HB 3564?
It covers the page-one fee list and the utilities statement. You also need to disclose required fees in listings and remove banned fees. Have your full lease reviewed by an attorney.
Can I attach it as an addendum?
No. Required fees must be on the first page of the lease. Make this the first page or copy the table onto it.
